Enrollment

When a spot becomes available at The Children's Nest, someone will call or email you. If you choose to accept the spot, you will need to pay a deposit equal to the first two weeks of care (which will then be applied to the first two weeks enrolled) and a registration fee ($30.00 for one child, $50.00 for two children, and $10.00 for each additional child). This deposit is non-refundable should you choose not to enroll your child at The Children's Nest.

Once you have accepted a spot at The Children's Nest, you will need to fill out all of the paperwork below. The physical form and shot record must be filled out by a doctor and can be faxed to us.

Please note: All lines of paperwork must be filled out. If something is not applicable, please put a dash through it or write "N/A." Emergency contacts must be local and we must have their full address.

All paperwork must be turned in before your child's first day.

Medical Authorization Form This form is to be filled out by enrolling parents. It requests information for Children's Nest to contain immediate medical care. Download here.

Parental Agreement & Identity Verification The first half of this document is a list of parental agreements and the second half is verification of a child's identity through proof of birth certificate. Download here.

Application for Enrollment This form is used to enroll a child in Children's Nest after they have been accepted from the waitlist. Simple information to include is the start date, and people names with contact information. Download here.
